Hearing Exams and Aids

In addition to providing eye exams and glasses at no cost to the needy in our community, we also provide hearing exams and hearing aids. For services Outside of the Fort Lauderdale area.

If you require a hearing exam or hearing aid(s) or both, please send a letter including the information below to: 

Fort Lauderdale Lions Club 
Attn: Hearing Chair 
P.O. Box 564 
Fort Lauderdale, FL 33301 

This letter MUST include the following information for the person requiring assistance: 

1. Name 

2. Address, including street, city and zip 

3. Telephone number - if you do not have a telephone, please provide a number where we can reach you. 

4. Social Security Number (SSN) 

5. Date of Birth (MM/DD/YY) 

6. Reason assistance is required 

7. If you have a prescription, the date of it and name and phone number of Audiologist. 

8. Name of person/agency referring you to the Lions
