Eye Exams and Glasses

Our primary project is providing eye exams and glasses at no cost to the needy in our community. For services Outside of the Fort Lauderdale area.

If you require either an eye exam or glasses or both, please send a letter including the information below to: 

Fort Lauderdale Lions Club 
Attn: Sight Chair 
P.O. Box 564 
Fort Lauderdale, FL 33301 

This letter MUST include the following information for the person requiring assistance: 

1. Name 

2. Address, including street, city and zip 

3. Telephone number, if you do not have a telephone, please provide a number where we can reach you. 

4. Best time to reach you. 

5. Social Security Number (SSN) 

6. Date of Birth (MM/DD/YY) 

7. Reason assistance is required 

8. If you have a prescription, the date of it, and the name of the Doctor that issued the prescription. 

9. Name of person/agency referring you to the Lions
